Instructions for Submitting a Request for Administrative Review

  • Complete the Citizen Request for Administrative Review form in English (below) or en Español.
  • Review Request must be submitted within 15 days of the citation date
  • Provide a mailing address and email address
  • Attach all supporting documentation with the review form 
  • A Decision Letter will be mailed to you

Please note: Pictures, evidence proof of payment, etc., must be submitted at the time of appeal. It is your responsibility to attach all necessary documents to ensure a fair and thorough review is completed.
