Instructions for Submitting a Request for Administrative Review
- Complete the Citizen Request for Administrative Review form in English (below) or en Español.
- Review Request must be submitted within 15 days of the citation date
- Provide a mailing address and email address
- Attach all supporting documentation with the review form
- A Decision Letter will be mailed to you
Please note: Pictures, evidence proof of payment, etc., must be submitted at the time of appeal. It is your responsibility to attach all necessary documents to ensure a fair and thorough review is completed.
