Coming Home Connection
Senior Community Care: Service navigation for most at-risk and in-need seniors. Free and affordable home care for seniors, including respite services for family caregivers. Also includes an equipment loan program for bedside commodes, hospital beds, walkers, wheelchairs, etc. at no cost to participants.
Esperanza Shelter
Community Navigation Services: The program will be a new service for all adult clients residing in Esperanza Shelter and for clients who have left the shelter and entered the transitional housing program. The Community Navigator will help clients define personal goals and then connect clients with community services, removing any barriers to those services that may exist.
Feeding Santa Fe
Targeted Recipient Program: Feeding Santa Fe is a drive-thru pantry that provides fresh food to families and individuals, including seniors on a fixed income and veterans.
Food Depot
Navigation Services: Food Depot provides hunger relief by acquiring and distributing food to people experiencing food insecurity through a combination of direct service and support from a network of nonprofit hunger relief partners. As the food bank for nine counties, the Food Depot provides food to over 145 nonprofit hunger relief programs, serving 57,000 people per year, and providing 430,000 meals each month.
Interfaith Community Shelter Group & ICS Woman's Summer Shelter
Interfaith Community Shelter Navigation Services: The mission of the Interfaith Community Shelter is to provide safe, hospitable shelter, food, and clothing to men, women, and children experiencing homelessness in the City of Santa Fe, and to promote self-reliance by providing a point of entry to the other services necessary to make the transition from homelessness to stable housing.
Kitchen Angels
Kitchen Angels Navigation Services: Reducing the Impact of Poverty on the Health of Homebound, low income, and Chronically Ill Individuals; Delivery of specialized meals to chronically or terminally ill clients with the goal of keeping individuals out of institutional care and instead keeping them in their own homes.
La Familia Medical Center
Diabetes and Obesity Prevention Program: Provide community and home-based primary/secondary preventative services aimed at-risk and high-risk families to improve support services and education for our patient population and their families living with obesity. Many of the services provided by this program are uncompensated by patient revenue, either because patients are uninsured or because services like home visits are community health worker support are not reimbursed by insurance.
Life Link
Navigation for Homeless Persons: Provide agency collaboration, street outreach, screening, and navigation services in Santa Fe to homeless individuals and families. The Navigator will assess for all social determinants of health and help our homeless residents navigate the complex system of care available in Santa Fe. The Navigator will coordinate with other agencies, when necessary referring clients to services elsewhere via the AHC network and closing referral loops for homeless clients referred to the Life Link.
Literacy Volunteers of Santa Fe
Basic Literacy and ESL Tutoring for Young Adults and Adults: Free tutoring to adults in reading, writing, and speaking English to strengthen our community, families, and workforce. With the shared goal to help people in Santa Fe achieve their fullest potential.
New Mexico Immigrant Law Center
Legal Services: Legal Immigration Services for Victims of Domestic Violence and Related Sexual Assault and Human Trafficking: In partnership with SOLACE Crisis Center will continue and expand legal immigration services to victims of domestic violence, and related sexual assault and human trafficking, in Santa Fe.
Santa Fe Dreamer's Project
Legal Services: Offers critical DACA-related legal services to 181 individuals and Green Card-related services to 23 individuals in the course of 1 year all with zero cost to the clients.
St. Elizabeth Shelter
Homeless Emergency and Navigation Services: Provide homeless individuals and families with emergency shelter, food, case management, counseling, life skills, disbursements of supplies, and referrals to other appropriate agencies for healthcare, employment, benefits, drug, and alcohol treatment, all while helping them get into housing.
YouthWorks
YouthWorks! Navigation and Young Adult Services: Program focus is to impact social skills; foster leadership and community engagement; teach positive behavior, communication skills, and responsibility through classroom and hands-on education leading to employment. Young adults receive wrap-around support including no-fee counseling and intensive navigation support services to assist in overcoming personal, familial, legal, and social obstacles. Programming includes paid hands-on job skill training combined with GED coursework and trades/professional certification and college readiness.
